• Click Register to create a username and password.
• Click Forgot Password to reset your password.
• Click Forgot Username to retrieve your username.

• Use this form to create a username.
• All fields marked with an asterisk (*) are required.
• Passwords must contain at least 8 characters with 1 uppercase letter, 1 lowercase letter, and 1 number.
• Once you click Register a verification message will be sent the email account you provide, which will allow you to activate your account.
• Once activated, you will be able to login on the login screen.

Under My Farms

• To get started, click on the Manage Farms link to add your farm.

Under My Info

• Your Agent Code is for reference only, and is your own unique number which will be assigned to all farms you create.
• Click Update my Info to edit your username details.
• Click Change Password to change your login password.

• Click on Manage Farms, then New Farm to add your farm.
• All fields marked with an asterisk (*) are required.

Farm Name

• This is the legal name of the farm business
• If your farm goes by a second name, tick the box and enter the secondary farm name.

Farm Address

• This is the physical address of the farm. All addresses should match the listing on VT E911 Viewer: http://maps.vermont.gov/e911/Html5Viewer/?viewer=e911viewer

Primary Contact

• The person who official correspondence should be addressed to, aka the business owner.

Mailing Address

• The mailing address is where official correspondence should be sent to.

Farm Media

• Enter the farm’s website, email, Facebook, or farm phone number.

Other Info

• Follow the series of questions to help identify whether your farm is subject to the Produce Safety Rule (PSR).
• Depending on the answers provided, a member of the VT Produce Team may follow up with you to confirm you farm’s status under the PSR.

On Farm Readiness Reviews (OFRR)

• Check this box to inquire about an educational on-farm assessment of produce safety risks for your farm.

• Farm information can be edited and resubmitted at any time. Simply login to the VT Produce Portal and click Manage Farms.
• Select the correct farm and click Edit. Once edits are made, make sure to click submit for changes to be saved.
